PETS & PEOPLE CO WAIVER FORM
I understand that due to the way that pets interact with one another, minor injuries can occur even though the pets are carefully supervised at all times. If I am unreachable in the event of an emergency, whilst my pet(s) is/are in the care and custody of Pets & People Co, I hereby authorize Pets & People Co. to seek immediate veterinary care for my pet. I understand that all costs in connection with veterinary or other treatment shall be my responsibility. I hereby entrust the keys to my property and the care of my pet to Pets & People Co. and I understand that Pets & People Co. will not be liable for claims in relation to legal actions, loss, injury or damage of any nature, which I or my pet(s) may sustain or which may be caused in any way by my pet(s). I specifically, without limitation, agree to fully indemnify Pets & People Co. for any and all such liability for claims in relation to legal actions, losses, injury or damage.
I certify that I have read and understood the terms and conditions of Pets & People Co. services, and confirm the truthfulness of the contents of the application form completed by me.
Although Pets & People Co. carefully screens all pets, occasionally we discover that this is not an appropriate partnership. Pets & People Co. reserves the right to discontinue any service with approprioate verbal or written notice.
